California PHC

PHC California members have the choice of either a gym membership at 24 Hour Fitness OR up to $200 worth of over-the-counter (OTC) pharmacy items every calendar year for no cost.

To make or change your Health and Wellness Benefit option election, contact Member Services; or download the Health and Wellness Benefit Option Election form, complete it and send it to Member Services. If you are continuing your enrollment in PHC California to a new calendar year, you may change your benefit option election, i.e., from a gym membership to OTC items or vice-versa, once a calendar year between January 1 and January 15. (A calendar year is January 1 through December 31.)

The 24 Hour Fitness membership may be used at any 24 Hour Fitness facility except Ultra-Sport locations. To find a gym closest to you, go to the 24 Hour Fitness website. (By clicking on this link, you will be taken to a website operated by 24 Hour Fitness and not PHC California.)
