Florida PHP

Although Medicare is a Federal program, PHP (HMO SNP) is available only to individuals who live in our plan service area.  To remain a member of our plan, you must keep living in this service area, which is Duval, Broward and Miami-Dade Counties. 

We also offer PHP in Los Angeles County, California and Fulton County, Georgia.  However, there may be differences between the plans we offer in each state.  If you move out of Duval, Broward or Miami-Dade County to Los Angeles County, California, or Fulton County, Georgia, and you wish to remain enrolled in PHP, you must call Member Services so we can arrange for you to transfer your enrollment.

If you move to a county or state outside of our service area, you cannot remain a member of our plan.  Please call Member Services so we can assist you with disenrollment from the plan.  When you move, you will have a Special Enrollment Period that will allow you to switch to Original Medicare or enroll in a Medicare health or drug plan that is available in your new location.
